SUPPORT AT HOME
Helping make Support at Home work at home
The right support doesn’t stop at the front door. For many older Australians, the home environment can become a barrier to safety, mobility and independence.
A step at the entrance, an inaccessible bathroom, difficult stairs or a lack of suitable support can make everyday activities harder – and can affect a person’s confidence to remain living at home.
Home Access Group helps Support at Home providers address these challenges through OT-led assessment and practical home modifications.
Our role is to understand the participant, assess their home and help deliver appropriate solutions based on their assessed needs.
SUPPORT AT HOME
Assistive technology & home modifications
Helping participants access the changes they need.
The Australian Government’s Assistive Technology and Home Modifications (AT-HM) scheme provides Support at Home participants with separate funding for approved assistive technology and home modifications to help them live safely and independently at home.
Home modifications can include changes such as:
- Internal and external handrails
- Grab rails
- Ramps
- Stair lifts
- Bathroom accessibility modifications
- Widening doorways and passages
- Changes to improve access around the home
- Other approved modifications based on assessed needs
The current AT-HM funding tiers for home modifications are Low – up to $500, Medium – up to $2,000, and High – up to $15,000, with the funding period generally being 12 months. Complex high-tier home modifications may have the funding period extended where the relevant requirements are met.
We help turn that funding into reality.

PARTNERS
Our Support at Home services
One partner from assessment through to installation. Home Access Group can support providers across the home modification process.
Occupational Therapy home assessments
We assess the participant and their home to understand their functional needs, goals, mobility and the challenges they are experiencing.
Our assessment considers how the person uses their home and what changes may help them remain safe and independent.
Home modification recommendations
We identify practical modifications that may address the participant’s assessed needs.
Recommendations are tailored to the individual and the home rather than based on a one-size-fits-all approach.
Professional reports & documentation
Clear documentation helps providers understand what is required and why.
We provide reports and recommendations that can support the decision-making, approval and implementation process.
Quotes & modification coordination
Where modifications are approved, we can assist with coordinating the practical requirements of the work, including obtaining appropriate quotes and coordinating the relevant trades or suppliers.
For more complex modifications, the AT-HM guidelines recognise the importance of detailed scopes of work and coordination between occupational therapists and building professionals.
Supply & installation
We can coordinate the supply and installation of approved home modifications through appropriate trades and installation professionals.
This may include rails, ramps, bathroom modifications, access solutions, lighting and other approved modifications.
Completion & follow-up
We keep providers informed throughout the process and provide appropriate completion documentation once work has been undertaken.
Our goal is to make the process straightforward for the provider and meaningful for the participant.

working together
For Support at Home providers
Home Access Group is the reliable partner for home modifications, helping reduce the complexity of coordinating home modification services for your participants.
We combine professional assessment with practical implementation to help providers deliver appropriate outcomes efficiently.

Supporting your participants
Every participant has different needs. A home modification might be required because someone is:
- Ageing at home
- Experiencing reduced mobility
- At risk of falls
- Recovering from injury
- Returning home from hospital or rehabilitation
- Living with disability
- Experiencing changing functional needs
- Planning ahead

Working with your care team
We understand that home modifications are part of a bigger picture. A modification rarely exists in isolation.
A participant may also be receiving occupational therapy, physiotherapy, nursing, personal care, domestic assistance or other services.
Home Access Group can work alongside the participant’s existing care team and Support at Home provider to ensure recommended home modifications are considered in the context of the person’s broader needs and goals.
The Support at Home program is designed around each participant’s assessed needs, goals and preferences, with providers responsible for coordinating and delivering appropriate services
