OT Home Assessments and home modifications to increase safety and reduce risk of falls

We work with providers of Support at Home and NDIS to support participants and all Australians with access to Occupational Therapy (OT) Home Assessments so they can remain independent and safe in their homes.

Which best describes you?

Refer a participant and let us coordinate the OT Home Assessment – prioritising care, communication and understanding toward their needs while keeping you informed at every step.

Our project management processes elevate communication, messaging and reporting at key stages from booking through to report and recommendations delivery.

Make your home work better and safer for you again

An Occupational Therapy (OT) Home Assessment can identify practical, affordable changes that help you stay safe and independent at home.

Whether you are senior, rehabilitating from injury, experiencing reduced mobility, or have changing health or functional needs, an OT Home Assessment can ensure safety solutions are tailored specifically for your needs.

A safer home can mean more independence

As we age, the home that once worked perfectly can become harder to navigate. A step becomes a trip hazard. A bathroom becomes difficult to use. Getting in and out of a favourite chair becomes a daily challenge.

Home Access Group brings together occupational therapy assessment, practical recommendations and home modifications to help people keep living safely and independently in the home they know.

Assess. Recommend. Modify. Access.

The OT Home Assessment process

Critical safety risk identification

When an OT Home Assessment identifies a critical safety risk, it is flagged and escalated for immediate remediation.

Prevention of harmful falls can be greatly reduced and eliminated when a potential hazard can be identified, flagged, escalated and removed.

A few practical changes are often all it takes to keep living safely and independently in the comfort of your own home.

An OT spots the hazards you’ve stopped noticing – the ones that only become obvious after someone’s hurt.

Clear communication at every step

Built on occupational therapy expertise, not a sales target

OT-led, always

Every assessment and recommendation is made by an occupational therapist working within their AHPRA scope of practice – not a product installer working backwards from a sale.

Fluid in AT-HM

We work to the Assistive Technology and Home Modifications scheme’s documentation, scope-of-work and reporting requirements, so what you receive is ready for your approval process.

One point of contact

A single contact for your team from referral through to completion – no chasing between an assessor, a builder and an installer.

Feedback fuels continuous improvement

We have built a robust project management workflow that incorporates feedback loops from partners and participants to ensure our service is continuously best-in-class.

Frequently asked questions

For older Australians and families

Make the home work for you – not the other way around.

You shouldn’t have to leave the home you love simply because your needs have changed. A professionally assessed home can identify practical changes that help you move around more safely, reduce the risk of falls, use your bathroom with greater confidence, and keep doing the everyday activities that matter to you.

Sometimes relatively small changes can make a significant difference to safety and independence. A professionally assessed home can identify practical changes that may help you:

  • Move around your home more safely
  • Reduce the risk of falls
  • Use your bathroom with greater confidence
  • Get in and out of your home more easily
  • Navigate stairs and changes in level
  • Continue doing the everyday activities that matter to you

Your home. Your independence. Your choice.

Our goal isn’t to change the way you live. It’s to help make your home work better for the way you live now – and into the future.

Our values

We are guests in someone’s home.

A home is personal. It is where people have built their lives, raised families and created memories.

We treat every person, family and home with respect – protecting privacy, listening carefully and ensuring people remain involved in decisions about their own home.

Better access. Greater independence.
A safer place to call home.

At Home Access Group, we believe the right changes to a home can make a meaningful difference to someone’s ability to live the life they choose.

That’s why everything we do is guided by a simple principle:

Make the home work better for the person who lives in it.
