Occupational therapy home assessments for providers

When a Support at Home or NDIS participant is experiencing challenges moving safely and independently around the home, finding an experienced and reliable Occupational Therapist to visit, understand the individual’s needs, and complete a home assessment is becoming increasingly difficult due to high demand.

Home Access Group makes the process easier and more accessible.

We coordinate OT Home Assessments in partnership with providers, supporting the end-to-end process from referral to assessment to recommendations.

We’re here to help Australians live in safer, more accessible homes.

How can we help?

We can arrange an occupational therapy-led home assessment to identify potential risks and practical opportunities to improve safety and independence.

A simpler way for home modifications.

Support at Home providers have responsibility for arranging and sourcing required AT-HM items and services. The government allows registered providers to engage third-party organisations to provide assistive technology and home modifications.

How we work

We can arrange an occupational therapy-led home assessment to identify potential risks and practical opportunities to improve safety and independence.

No unnecessary back-and-forth. No need to source an OT. And no need to wait several months for OT availability.

Support at Home providers have responsibility for arranging and sourcing required AT-HM items and services. The government allows registered providers to engage third-party organisations to provide assistive technology and home modifications.

Home Access Group provides a specialist partner that can help manage this process.

1. Refer

Home Access Group receives the participant referral and relevant information. We’ll review the referral and confirm the appropriate assessment pathway.

2. We coordinate the OT Home Assessment

We arrange a suitably qualified OT to conduct a Home Assessment. We manage appointment coordination and provide clear and transparent communication with the participant with a focus on prioritisation around their availability and living situation.

3. The OT assesses

The OT considers the participant, their goals, functional needs and home environment. The assessment can consider areas such as:

  • Mobility and transfers
  • Falls and safety concerns
  • Bathroom access
  • Stairs and entrances
  • Bedroom and nighttime access
  • Kitchen and daily activities
  • Wheelchair or mobility equipment access
  • Other barriers around the home

4. Receive the recommendations

The OT provides their assessment and recommendations. Depending on the referral, this may include recommendations for home modifications, assistive technology or other practical strategies.

5. You decide what happens next

The provider receives the assessment and recommendations to support the next agreed steps with the participant.

More than just sourcing an OT

We coordinate the complete Home Assessment process.

Finding an OT is only one part of arranging an assessment.

The end-to-end process also includes:

Referral → Information gathering → OT matching → Appointment → Participant communication → Assessment → Report → Recommendations → Provider follow-up

Home Access Group helps coordinate that journey. That means your team can spend less time chasing appointments and paperwork, and more time supporting participants.

We don’t start with a product. We start with a focus on the person.

A home modification shouldn’t simply be about installing a grab rail or changing a bathroom. It should begin with understanding:

What is the participant trying to do? What has become difficult? What would they like to keep doing? What is making that harder?

A Home Access Group OT Home Assessment considers the person and their environment together, helping identify practical solutions that support safety, function and independence.
