Helping make homes safer, more accessible and easier to live in.

At Home Access Group, we provide occupational therapy-led home assessments, recommendations and home modifications designed to help people remain safe, independent and at home for longer.

From identifying potential hazards to installing practical modifications, we can help manage the process from assessment through to completion.

Occupational Therapy Home Assessments

Understanding the person. Understanding the home. A home assessment looks at more than just the physical environment.

Our occupational therapy approach considers the individual, their abilities, their daily routines and the activities they want to continue doing – and then looks at how the home can better support them.

An assessment may consider:

  • Mobility around the home
  • Risk of falls
  • Transfers and movement
  • Bathroom and shower access
  • Stairs and changes in level
  • Entry and exit from the property
  • Kitchen and household activities
  • Lighting and visibility
  • Bedroom access
  • Current and future mobility needs
  • The person’s goals and priorities

The result is a clearer understanding of what is working, what may present challenges and what changes could make the home safer and more accessible.

Identify risks before they become bigger problems. Many hazards around the home aren’t immediately obvious. A home safety assessment looks at the environment from the perspective of the person living there and identifies potential hazards, barriers and opportunities for improvement.

We can assess areas including:

Entry & exit
Steps, thresholds, paths, doors and access points.

Living areas
Furniture placement, movement pathways, seating and trip hazards.

Bathroom
Shower access, transfers, flooring, rails, seating and other safety considerations.

Bedroom
Bed access, transfers, lighting and movement around the room.

Kitchen
Access to appliances, benches, storage and frequently used items.

Stairs and levels
Stairways, steps, handrails and changes in floor level.

Lighting
Visibility, shadows and lighting requirements around key areas of the home.

Home modification assessments

The right modification starts with the right assessment. Not every person needs the same solution. Our recommendations are based on the individual’s needs, the characteristics of the home and the activities they want to continue doing.

Depending on the assessment, recommendations may include:

  • Grab rails and support rails
  • Handrails
  • Ramps
  • Threshold modifications
  • Bathroom modifications
  • Shower access solutions
  • Shower seating
  • Toilet access modifications
  • Improved lighting
  • Changes to doorways and access
  • Stair safety solutions
  • Mobility access improvements
  • Other appropriate home modifications

Where required, we can provide documentation and reports to support the assessment, recommendation and approval process.

Our approach

Home modification recommendation

From understanding the individual’s circumstances, lifestyle and challenges, our Occupational Therapy home assessments provide recommendations to help maintain independence in a safe home environment.

If required, Home Access Group can assist coordination of the installation of approved modifications, working with appropriate trades and professionals to ensure recommended solutions are put into place.

Strategically positioned rails to provide additional support around the home.

Solutions to improve access over steps, thresholds and changes in level.

Practical changes designed to improve safety and accessibility in one of the most important areas of the home.

Improvements to visibility in entrances, hallways, stairs and other areas where additional lighting may be beneficial.

Changes to doors, thresholds and other areas that may create barriers to movement.

We can assist with a range of home modifications based on the individual’s assessed needs.

One partner from assessment to installation

Every home is different. Every person is different. We don’t believe in a one-size-fits-all approach. Our recommendations take this into account and we understand the most effective solution isn’t simply about installing a product.

It’s about making the home work better for the person who lives in it.

The person

Their abilities, mobility, goals and preferences.

The home

Its layout, construction, access points and existing features.

The activity

What the person wants or needs to be able to do.

The future

How changing needs may affect the home over time.

Families and individuals

Concerned about someone you know and care about? You may have noticed that Mum is struggling with the stairs.

Dad may have had a fall in the bathroom. Or perhaps getting in and out of the house is becoming more difficult. These changes don’t necessarily mean it’s time to leave home.

A professional OT Home Assessment can help identify practical changes that may make everyday life safer and easier.

Support at Home providers

A reliable partner for home modifications. Home Access Group can help reduce the complexity of coordinating OT Home Assessments and home modification services for your participants.

We combine professional assessment and transparent communications with practical implementation and reporting that help providers deliver appropriate outcomes more efficiently.

Helpful answers before you get started

A home modification is a change to the physical environment that helps improve a person’s safety, accessibility or independence at home.

Depending on individual requirements, modifications may include grab rails, handrails, ramps, bathroom modifications, lighting, access improvements, threshold solutions and other recommended changes.

An OT assessment can help determine what modifications are appropriate based on the individual’s needs and circumstances. Under the Support at Home AT-HM scheme, home modifications must be prescribed by an occupational therapist working within their professional scope of practice.

Yes. Registered Support at Home providers may engage third-party organisations to deliver services, assistive technology and home modifications. The registered provider remains responsible for ensuring third parties meet applicable requirements.

Following assessment and approval, we can assist with obtaining quotations and coordination of the supply and installation of appropriate home modifications

Our services are particularly suited to older Australians and people experiencing changing mobility or accessibility needs. However, home modifications can benefit anyone who requires greater accessibility or independence at home.

Let’s make home access easier.

Safer. More accessible. More independent.

Whether you’re looking to make your own home safer, helping a family member remain independent, or seeking a reliable home modification partner for your Support at Home participants, Home Access Group can help.
